It's a good list but maybe in the health questions you should put something like "Has the bird been tested for common diseases such as psittacosis?" being as it is carried in 60% of birds! I recommend that if a bird hasn't been tested for it, then you do test for it. It's what? 25 with a vet here and 13 to send it off. It's better to be safe than sorry and end up forking out $$$$$ on treatment if it can be prevented so yeah, asking them if the bird has been previously tested would be good

Another thing would be in the behaviour bit would be to ask if the bird was cage aggressive or not. Most birds like their cages to be left open so they can come out on their own terms rather than you trying to get them to step up

Daisy - thanks for replying to this!!

I thought about asking about diseases like psittacosis but I decided against it at first,
just because I didn't think it was super likely that too many people have had their
birds tested for anything. I mean, it would be nice to think that every bird owner (or
a majority of them) takes their birds to the vet. I really don't think that many people
do. However, my goal is to be as thorough as possible, so I'll go ahead and add that

I'll also add the cage aggressive thing in. Thanks again!
